Anyone living with ME/CFS, Long Covid or POTS knows the moment: you try to explain why a short walk or a phone call costs you the rest of the day – and you see nothing but polite confusion on the other person's face. Because the exhaustion caused by post-exertional malaise (PEM) has so little in common with ordinary tiredness, many people reach for vivid comparisons. They make the invisible tangible, if only for a moment. But not every image fits every situation, and some comparisons can – unintentionally – create false expectations. This article introduces the most common comparisons and shows where they are helpful and where caution is warranted.
The Spoon Theory: Origin and Core Idea
The best-known comparison is the so-called spoon theory. It describes how people get by on a limited supply of energy: spoons stand for the amount of physical and mental strength available on a given day — and that supply can run out. The term was coined in a 2003 essay by American author Christine Miserandino, in which she used a handful of spoons as a metaphor for units of energy to describe her experience with a chronic illness.
Applied to everyday life, this means: every activity – showering, getting dressed, going shopping, having a conversation – costs a certain number of "spoons". Other Images That Can Help in Everyday Life
The spoon theory is not the only comparison – and depending on who you're talking to, a different image may work better. A commonly used example is the phone battery: healthy people recharge to 100 percent overnight, while for someone with chronic fatigue the battery charges more slowly and drains more quickly. The image of an "energy envelope" or energy budget, used in rehabilitation and physiotherapy, works similarly: pacing is often described as trying to stay within an energy "envelope", or explained using ideas like the amount of "battery" or "spoons" a person has available for activities or tasks.
For children and teenagers with Long Covid, there are even child-friendly variants of such images. A British patient organisation, for instance, uses a penguin metaphor, in which each fish eaten represents one unit of energy – an image that is often more accessible to younger family members than abstract explanations. Such age-appropriate comparisons show that it's worth adapting the image to the person you're trying to explain something to – partners, colleagues, children or grandparents often need different approaches.
Where the Comparisons Reach Their Limits
As helpful as these images are, it is just as important to know their limits. The spoon theory, for example, is purely a thought experiment with no scientific testing of its effectiveness: no studies have so far been conducted on the effectiveness of this analogy. That doesn't make it wrong, but it does mean it is a communication aid, not a medical concept.
Another common misunderstanding arises from the image of the battery or the spoons being refilled overnight: it suggests that the supply simply replenishes itself after a night's sleep – similar to how healthy people recover after a tiring week. With PEM, that's not how it works. A crash may not appear until one to three days after exertion, and can then last for days or longer, rather than "recharging" overnight. Anyone using a comparison should ideally explain this point as well – otherwise it can create the impression that a good night's sleep would be enough to "recharge".
The language of "rationing" or "budgeting" can also unintentionally suggest that pacing is purely a matter of willpower or organisation – as if smarter planning alone would solve everything. In reality, available energy fluctuates from day to day, sometimes even from hour to hour, and cannot always be predicted. A comparison should therefore never stand alone, but should always be supplemented with one's own, concrete experience: how many "spoons" does brushing my teeth, a phone call, or a doctor's appointment personally cost me? Concrete examples from everyday life make any image more credible than the theory alone.
What Really Helps in Conversation
Drawing on the experience of many affected people and patient organisations, a few recommendations emerge. First: one image is usually enough to open a door – the details can follow afterwards. Second: comparisons work best when they are linked to a real, small situation rather than remaining abstract. For family and friends, such a comparison offers a way to show empathy and active support without those affected having to explain themselves at length or even prove anything. That is often the real goal of a conversation: not persuasion, but a shared piece of language.
Third: repetition is normal. A single conversation rarely changes understanding permanently, especially when symptoms fluctuate and are not visible from the outside.
