Not all Long COVID fatigue is the same — but anyone experiencing PEM should understand pacing before the next activity increase turns into a setback.

Long COVID (also called post-COVID condition or PASC) describes persistent symptoms after a COVID-19 infection. A subset of those affected develop a symptom picture that closely resembles ME/CFS — including post-exertional malaise (PEM): a delayed worsening after exertion, often 12 to 72 hours later. For this subgroup, pacing is the most important non-drug strategy for avoiding setbacks.
Many people with Long COVID are initially given the well-meaning advice to gradually increase their activity. For the subgroup with PEM, however, this can lead to repeated crashes and a gradual decline. US and UK health authorities (CDC, NICE) now recommend checking first whether PEM is present — and if so, starting with pacing rather than activity progression.
The pacing calculator gives instant, free guidance on your daily exertion limit. If you notice a racing heart or dizziness when standing up, the guided stand test (NASA lean test) can generate a structured finding for your doctor's visit. The app's daily tracking view links wearable data (Garmin, Apple Health, Whoop, Oura and more) with symptom entries to reveal exertion patterns over several days — or works entirely without a device, using manual entries alone.
Note: mypacing is not a medical device and does not replace medical diagnosis or treatment.
